Usability Activity Log v2.3 is the easiest and most economical way to document observational data.  A necessary tool for any corporate usability lab.

Usability Activity Log was originally created for Target Corporation back in 1997.  After it's initial release, it was quickly discovered by other top organizations within the usability community by word of mouth.

Bit Debris Solutions was originally founded as Bit Debris BBS, a dialup bulletin board system, back in 1991.  The domain name bitdebris.com was first registered in 1992 once the Internet and UUCP email transport came on to the scene.  Bit Debris BBS went offline in 1995 only to come back online in 1999 on what we now call the World Wide Web.
